We are currently welcoming our regular patients and also have limited availability for new patients who are ready for a change in primary care.

If you are a current or previous patient of Carol, Leah, or Eric, you’ll need to complete a Release of Information (ROI) consent form so your medical records can follow you to HighTide Healthcare. Once completed, sign the form and email it to:

Even if you’ve been seeing one of our providers for years, this step is still required as we open our new practice.

REACH OUT
Please submit your completed Release of Information form first. Once that’s done, give us a call to schedule your visit. After your appointment is scheduled, you’ll receive instructions to join our patient portal and complete the setup process. Our team will walk you through next steps, help finalize any remaining paperwork, and make sure everything is in place as we prepare to open.
